30 Years of Service
Founded in Reseda, CA
All-Med Home Health was established as a small, community-focused home health agency in Reseda, California, with a commitment to serving Medicare beneficiaries in the San Fernando Valley.
Medicare Certification
Received full Medicare certification (Provider #058027) from the Centers for Medicare & Medicaid Services, enabling us to serve a broader population at no cost to qualifying patients.
Bilingual Services Launched
Recognizing the diverse needs of our community, we expanded our team to offer fully bilingual English and Spanish services β a critical bridge for many of our patients and families.
Expanded Service Area
Growth allowed us to extend our services throughout greater Los Angeles County, including Northridge, Encino, Tarzana, Woodland Hills, and surrounding communities.
Quality Recognition
Achieved a 0.0% patient fall rate β a testament to our comprehensive safety protocols and dedicated clinical staff. Our medication management rate reached 84%, well above the national benchmark.
