1Medicare & Eligibility

Yes. Medicare Part A covers home health services when you meet eligibility criteria. To qualify, you must: (1) be under the care of a doctor, (2) receive services under a plan of care established and periodically reviewed by a doctor, (3) need skilled care (skilled nursing, physical therapy, speech therapy, or occupational therapy), and (4) be homebound. When eligible, Medicare covers 100% of approved home health services with no copay β€” there is no cost to you.
Medicare defines 'homebound' as having a condition due to illness, injury, or disability that makes leaving your home require considerable effort. You may still qualify if you occasionally leave for medical appointments, adult day care, religious services, or family events β€” the key is that leaving requires a significant effort. Examples of homebound patients include: individuals who need help from another person or device (walker, wheelchair, cane) to leave home; those whose physician advises them not to leave; patients recovering from surgery or illness who are temporarily restricted; and those with severe shortness of breath, cognitive impairment, or severe pain when moving. Getting started is easy: 1. Contact us by calling 818.986.2356 or filling out our online intake form at /contact. 2. We'll work with your doctor to obtain a referral/order for home health services. 3. A care coordinator will visit your home for a free assessment to understand your needs. 4. We verify your Medicare eligibility and coverage. 5. A personalized care plan is created with your doctor's input. 6. Care begins β€” typically within 24-48 hours of receiving physician orders. You can also ask your doctor, hospital discharge planner, or social worker to refer you directly to All-Med Home Health.

Our clinical staff β€” including nurses, therapists, and home health aides β€” can communicate with patients and families in both languages. We believe that receiving care in your primary language is essential for understanding your condition, following care instructions, and feeling comfortable and respected. If you or a family member is more comfortable in Spanish, simply let us know and we will match you with bilingual staff.

Yes. While Medicare is our primary payer, we also accept Medi-Cal (California Medicaid) and select private insurance plans. We also offer private-pay arrangements for patients who wish to receive services not covered by insurance. All-Med Home Health provides a comprehensive range of home health services: β€’ Skilled Nursing β€” Medication management, wound care, IV therapy, disease monitoring β€’ Physical Therapy β€” Mobility, strength, fall prevention, post-surgical rehab β€’ Occupational Therapy β€” ADL retraining, fine motor skills, cognitive rehab β€’ Speech Therapy β€” Communication and swallowing disorders β€’ Personal Care β€” Bathing, dressing, grooming, meal prep, companionship β€’ Wound Care β€” Surgical wounds, diabetic ulcers, pressure injuries β€’ Diabetic Care β€” Blood glucose monitoring, insulin management, foot care β€’ Post-Surgery Care β€” Recovery monitoring, pain management, medication education All services are delivered in your home by licensed, experienced professionals.
Visit frequency depends on your individualized care plan, your clinical needs, and your physician's orders. Some patients receive daily visits while others may receive care 2-3 times per week. Our care coordinator will work with you and your doctor to determine the appropriate schedule. As your condition improves, the frequency of visits typically decreases. We also offer 24/7 on-call nursing support for current patients with urgent concerns.
We do our best to maintain consistency in your care team and assign you the same primary nurse and/or aide whenever possible. Continuity of care is important to us β€” it builds trust and helps our staff get to know your individual needs. If you have specific preferences (e.g., bilingual staff, gender preference), please let your care coordinator know and we will do our best to accommodate you.
In a life-threatening emergency, call 911 immediately. For current All-Med patients, our 24/7 on-call nursing line is available after business hours for urgent clinical questions or concerns. Your nurse will have your physician's contact information and emergency protocols documented in your care plan. Our staff are trained to respond to emergencies and will communicate with your doctor and emergency services as needed.
Yes, this is one of our most common situations. Post-hospital discharge care is an excellent use of home health services. If your doctor determines that you need skilled care after leaving the hospital, they can write an order for home health. Many hospitals have discharge planners or social workers who coordinate directly with home health agencies like All-Med. We can typically begin service within 24-48 hours of hospital discharge with proper physician orders. Medicare may cover this at 100% if you meet eligibility criteria.

3Billing & Insurance

If you have Medicare Part A and meet all eligibility requirements, there is no cost to you for covered home health services. Medicare pays 100% β€” there is no deductible, no copayment, and no coinsurance for approved home health visits. This applies to skilled nursing, physical therapy, occupational therapy, speech therapy, and home health aide services when ordered by a physician. Medical social work services are also covered.
Medi-Cal is California's Medicaid program for low-income individuals and families. It provides free or low-cost health coverage including home health services. You may qualify for Medi-Cal based on income, age, disability status, or other factors. If you receive both Medicare and Medi-Cal (called 'dual eligible'), you may have even more services covered. Yes. All-Med Home Health handles all billing directly with Medicare, Medi-Cal, and accepted private insurance plans. You should not receive a bill for covered services. If any non-covered services are identified before we begin, we will inform you in writing so you can make an informed decision. We are committed to full transparency regarding any potential patient financial responsibility.
